Request For Quote- 5" steel spacer

I am looking for 2 very simple rings:

Material: 1018 (or similiar -- I can supply 5" round bar if needed)
Material quote requested
Qty:2
Time frame: I would like to have in a week.

For now I am many interested in quotes to create the shoulder, ID, and to face to the proper widths.

If someone is interested in drilling five 5/16" holes with chamfers and four 1/2" holes with counterbores into the outside flange, please provide a ball park cost adder and I can provide additional drawings for that feature.
